(5) A notice of appeal shall be in the format provided for
in Form B in the Seventh Schedule to these Rules and shall be
signed by or on behalf of the appellant.
89.
(1) A party intending to appeal shall, within fourteen
(14) days after lodging a notice of appeal:—
Service of

(a) serve copies of it on all persons who seem to him
to be directly affected by the appeal; but the Court
may, on an ex parte application, direct that service
need not be effected on any person who took
no part in the proceedings in the First Instance
Division; and
(b) file in the Registry an affidavit of service.
(2) Where any person required to be served with a
copy of a notice of appeal gave any address for service in or in
connection with the proceedings in the First Instance Division,
and has not subsequently given any other address for service, the
copy of the notice of appeal may be served on him at that address,
notwithstanding that it may be that of an advocate who has not
been retained for the purpose of an appeal.
(3) A notice of appeal shall not be incompetent by
reason only that the person on whom it is intended to be served
was deceased at the time the notice was lodged but the copy shall
be served as soon as practicable on the legal representative of the
deceased.
90.
(1) Every person on whom a notice of appeal is served
shall, within fourteen (14) days after service on him of the notice
of appeal:—
(a) lodge in the appropriate registry and serve on the
intended appellant notice of a full and sufficient
address for service;
